Event description
One of the workshops at MetalliKO was hit by an incident, forcing the plant to halt the entire steelmaking process. MetalliKO’s specialists spent three hours trying to locate and fix an equipment failure, which turned out to be caused by a broken connecting pin on the foundry crane’s lifting mechanism.
The plant’s chief engineer admitted that he failed to prevent the accident. But he emphasized that the plant uses a maintenance, repair, and operations (MRO) system so the whole set of planned inspections, regular diagnostics, and equipment maintenance was carried out strictly according to regulations.
However, a follow-up inspection revealed that the scheduled maintenance data stored in the MRO system was not accurate. The timeline and scope of work were adjusted by one of MetalliKO’s lead technical specialists, V. Petrov. But according to Mr. V. Petrov, he physically couldn’t change the schedule because he was on sick leave on the dates when the changes were made.
It’s still unclear what’s behind the production halt: human error or a hacker attack. But the plant has already suffered financial loses, while its management is starting to doubt whether the maintenance work was actually performed without violations.
